General Liability Waiver--Sciathlon

This permission includes all related programs or events associated with Villa Joseph Marie High School's Sciathlon. In consideration for my child's participation, I and my child understand that we assume any inherent risk, and with full knowledge of the risks, we agree to release and hold harmless Villa Joseph Marie High School and their employees and representatives, from claims arising or related to our (my) child’s participation.

Illness or Emergency

In case of illness or emergency, I authorize the officials of Villa Joseph Marie High School to contact directly the emergency contact persons named on this form. In the event that parents, physician, or other named on this form cannot be contacted, the school officials are authorized to take whatever action is deemed necessary for the health of my daughter.

Photo Release Consent

At Villa Joseph Marie High School, we may take photographs or videos of students participating in camp activities to share on our school’s social media, website, or marketing materials. Please indicate your consent for your child’s image to be used as described below
