By submitting this form, the parent/guardian consents to the child’s participation in the ELA Extravaganza at Villa Joseph Marie High School and acknowledges the following:
In the event of a medical emergency, emergency medical personnel may be contacted to assess and treat the student. If a parent/guardian cannot be reached promptly, Villa Joseph Marie High School (VJM) is authorized to secure appropriate emergency medical care, including evaluation, transportation, and hospital treatment. Families are responsible for all medical expenses incurred.
Students experiencing symptoms of illness, including fever or contagious conditions, should not attend. If a student becomes ill during the event, a parent/guardian will be required to arrange pickup.
VJM staff may provide basic first aid and non-invasive care as needed.
I permit VJM to use photographs or video of my child taken during event activities in promotional materials, social media, the VJM website, and school publications. Student names will not be published.
The ELA Extravaganza promotes respect, safety, and inclusion. Students are expected to uphold these values throughout the event. Behavior inconsistent with a safe and welcoming environment—such as endangering others, bullying, teasing, harassment, unsafe conduct, inappropriate language, or property damage—may result in removal from the event.
Participation in the ELA Extravaganza involves minor risks inherent in group activities. By signing, I release and hold harmless Villa Joseph Marie High School, its faculty, staff, and representatives from claims or liabilities arising from my child’s participation, except in cases of gross negligence or intentional misconduct.
