PIAA Form

All students who wish to participate in the athletic program at Villa Joseph Marie High School must submit a Phyiscal Evaluation Form from the Pennsylvania Interscholastic Athletic Association (PIAA).  The form can be downloaded below. 

The completed form needs to be returned to the school no later than August 9, 2012.
